Do I need to purchase more than one membership?
A. your Millcreek Paramedic Service membership only covers a portion of your out-of-pocket expense if you a Millcreek Paramedic Service ambulance transports you. If you are in any other areas often such as for work, you will need a separate membership for other services in the area if you want the services provided by them to be covered under their membership terms.
Q: What are the benefits of an MPS membership?
A: A reduction of $125 of your out-of-pocket expenses when ambulance transportation is provided and deemed medically necessary by YOUR insurance company, medical necessity meeting CMS (Medicare/Medicaid) guidelines.
If another ambulance service in Erie County transports a member of MPS for an emergency, MPS will pay the balance of your financial responsibility after insurance has been billed and collected. Medically necessary, non-emergency transportation within a 150-mile radius of Millcreek township, when non-emergent service is provided by MPS.
Q: Who is included in a “family membership”?
A: Anyone permanent resident in the household
Q: Is an MPS membership transferable, refundable or pro-rated?
A: No, the membership is non-transferable, non-refundable and cannot be pro-rated.
Q: Is my MPS membership tax deductible as a charitable contribution?
A: No, but it may be eligible as an itemized medical deduction.
Q: What is the cost of an MPS membership?
A:
Individual $50
Senior Individual (age 65 or older at time of purchase) $45
Family – includes everyone residing in the household $100
Senior family (includes up to 2 seniors age 65 or older at time of purchase residing in same household) $90
Q: When will my membership expire?
A: Our membership drive is from November 1 – December 31 each year. All memberships expire December 31st.You are able to purchase a membership outside of those times, however all memberships purchased between January 1 and October 31 will not be prorated and will expire December 31 of the current year.
Q: Does everyone in my family need a membership card?
A: We do not issue membership cards.
